Transitional Age Youth

Transitional Age Youth (TAY) Services focuses on improving outcomes for individuals aged 18-24 facing significant behavioral health challenges. The program aims to facilitate successful transitions to independence by collaborating with families, promoting self-sufficiency, and seamless integration into the community.

Program Details

TAY Services supports emerging adults who are experiencing or at risk of serious behavioral health challenges in making successful transitions to self-sufficiency and adulthood. Our network of contracted Community-Based Organizations (CBOs) provides developmentally appropriate behavioral health support and connection to resources.

TAY Services include:

  • Full-service partnerships
  • Clinical service teams
  • Early psychosis program
  • Outpatient programs
  • Triage, short-term case management
  • In-Home Outreach Team (IHOT)
  • Crisis residential programs (Age 18+)
  • TAY Residential Treatment Program (13 beds, Serves ages 18-24)
  • Prevention services serves ages 14-24

Full-Service Partnerships (18-24)

Our highest level of outpatient support provides a full spectrum of team-based behavioral health and community support services for TAY with exceptionally high needs living with serious mental health conditions.

Clinical Service Team (17-24)

Mental health and case management services for TAY living with serious mental health conditions.

Early Psychosis Program (15-24)

Early intervention mental health and community support services for youth/young adults 15-24 who have experienced their first episode of psychosis within the last two years.

Community Mental Health Outpatient Programs (16-24)

Mental health and case management services for TAY living with moderate to severe mental health conditions.

Triage, Short-Term Case Management (16-24)

Short-term (average 90 days) for TAY experiencing a behavioral health crisis and needing assessment and linkage to other programs.

In-Home Outreach Team (IHOT) (18-24)

Outreach and Engagement for TAY living with serious mental health conditions who are:

  • Reluctant to seek needed mental health services or
  • Were receiving services from an outpatient mental health provider, disengaged from services and need to be linked back.

TAY Residential TREATMENT Program (18-24)

A 24-hour supervised residential mental health treatment program for TAY who need additional support living independently in the community, especially those who are stepping down from a restrictive setting. Voluntary program, not a locked setting.

Prevention Services

Prevention and early intervention services for youth/young adults who may be struggling with emerging behavioral health challenges. These programs help recognize early signs of behavioral health concerns and prevent them from becoming severe and/or disabling.
