Age Group Served: Adults

Program Description: Provide intensive case management services to clients diagnosed with a severe and persistent mental illness (Schizophrenia, BiPolar, Major Depression). Serve clients in an integrated mental health and primary care setting. Work with an interdisciplinary team of social workers, psychiatrists, primary care doctors, peers and nurses to provide serves to the whole person. Learn Medi-Cal documentation, Wellness and Recovery principles and cutting edge principles of health integration.

Placement Period: 6-9 months

Program/Placement Hours: 8:30am-5pm

Preferred Discipline / Experience: Social work, mental health, health, consumer

First or Second Year Students: 2nd year students only

Supervision & Support: Weekly individual supervision, team meetings, ongoing trainings
