Quality Improvement & Accreditation

Quality Improvement (QI) serves as the through-line for department-level QI efforts, including sustained accreditation from the Public Health Accreditation Board (PHAB) and technical assistance resulting in programs and services with individually based or population approaches with measurable and meaningful impacts. Primary drivers for continuous improvement are embodied in the triannual Community Health Needs Assessment (CHNA), which directly informs Alameda County’s Community Health Improvement Plan (CHIP). QIA oversees departmental accreditation and processes that result in Alameda County’s Community Health Needs Assessment and Community Health Improvement Plan.

Alameda County Health, Public Health Department’s Strategic Plan

The Strategic Plan guides the Department’s role in optimizing population health by advancing social and health equity. In 2021-22, the Alameda County Health, Public Health Department engaged in a year-long process of updating our Strategic Map.

Community Health Needs Assessment and Community Improvement Plan

The Community Health Needs Assessment (CHNA) takes a broad view of health conditions and status in Alameda County, and the Community Health Improvement Plan (CHIP) articulates the long-term, system-wide effort to improve the Health of Alameda County’s communities.
