Diabetes Program

The Diabetes Program believes in getting to the heart of what the community needs and teaching the community how to deal with diabetes in the real world. The program offers education classes, support groups, and treatment plans to to assist in controlling diabetes.

The Diabetes Program offers self-management classes for adults diagnosed with pre-diabetes and Type 2 diabetes. Our team provides education, support groups, and personalized treatment plans to help you manage your condition effectively.

The Diabetes Program’s goals are to:

  • Prevent medical problems related to diabetes
  • Improve your understanding of Type 2 diabetes
  • Improve your ability to control your diabetes

The Diabetes Program offers:

  • Education classes, including the Diabetes Self-management Class for adults diagnosed with pre-diabetes and Type 2 diabetes
  • Support groups
  • Treatment plans to assist you in controlling your diabetes

Diabetes Self-Management Class

The Diabetes Self-Management Class is an eight-week course that meets once a week for 2 hours. Topics discussed include monitoring blood glucose, eating healthier, and more. 

Education Classes and Support Groups

Diabetes Support Group

The Diabetes Support Group meets once a month for two (2) hours. Topics vary. To learn more, please call the Diabetes Program at (510) 383-5185.

Diabetes Education Classes

We recommended that individuals attend ALL classes to completely understand and control diabetes.

The Diabetes Class series is an eight-week course. Eligible individuals meet once a week for 2 hours. Topics discussed are the following:

  • What is Diabetes? And monitoring blood glucose
  • Eating healthier — focus on carbohydrates
  • Eating healthier — focus on heart health, fat, and fiber
  • Physical activity
  • Taking medications
  • Reducing risks of complications
  • Coping with stress and emotions
  • Behavior change, problem solving, and resources

The Hypertension (HTN) Class series is on hold, however HTN one-time workshops can be scheduled. Topics discussed include the following:

  • What is Hypertension?
  • Nutrition
  • Physical activity
  • Lifestyle modification
  • Medications
  • Heart attack
  • Stroke

The Hypertension workshop goal is to:

  • Prevent medical problems related to hypertension
  • Improve understanding of hypertension
  • Improve ability to control hypertension

Diabetes Program Eligibility

Eligibility for the Diabetes Program includes:

  • Live in Alameda County.
  • Must be 18 years of age or older.
  • Must be diagnosed with pre-diabetes or Type 2 diabetes.

Individuals providing care or support for eligible clients are welcome to accompany their clients.
