MTP is a statewide program that provides occupational and physical therapy to children with certain physical medical conditions.

What is the Medical Therapy Program?

Therapists will work with you and your child to set therapy goals and make a plan to reach the goals.

How do I apply for medical therapy services?

Just like the CCS Case Management program, anyone can refer a child or youth to the Medical Therapy Program.

  • A medical provider can refer a child or youth by submitting reports that describe their patients’ medical eligibility and need for PT and/or OT. When the referral comes from a medical provider, CCS will send the young adult or family an application to complete.
  • Young adults and families can also submit an application. The application must be filled out and signed by a youth 18 or over or a child’s parent, legal guardian, or family member. CCS staff will review the application.
  • CCS will request reports from the child or youth’s doctor about the condition, injury and/or disability that may make them eligible for CCS. The reports will be reviewed by CCS to see if the child or youth is eligible.
