Message from the Director

We, as a system and as a community, are currently poised to make a significant difference in the serious challenges that Alameda County is facing. Since 2016, Alameda County has received $168 million across six years from the California Department of Healthcare Service to implement a whole person care pilot to “coordinate health, behavioral health, and social services in a patient-centered manner with the goals of improved health outcomes and more efficient and effective use of resources.”

With the resources of Alameda County Care Connect, we created an opportunity to deliver a more coordinated approach to care and services that will ultimately lead to optimal independence and health for our consumers.
