The mission of the Alameda County Health Care for the Homeless (ACHCH)/LifeLong Trust Health Center is to improve the lives of clients by providing high-value care that addresses clients’ physical, mental, and social well-being in one location. This collaboration between multiple government agencies and nonprofit organizations will promote dignity, respect, and compassion in all interactions among staff and clients. Participating organizations will collaborate closely to support clients in finding and maintaining stable homes, promote their overall health and well-being, and develop or re-establish strong social support networks outside of the public services system.
Primary care at the Trust Health Center is offered by LifeLong Medical Care under contract with ACHCH.

To become a client of the Trust Health Center, a person must be:
- An adult
- A resident of Alameda County
- Homeless or at-risk of homelessness OR receiving case management from the Alameda County Behavioral Health Department
