Eligibility and Contact Information

Eligibility for the Older Adults Health Results includes:

  • Be 60 years of age or older
  • Be eligible for full scope Medi-Cal
  • Have two or more medical conditions
  • Need help with at least one activity of daily living
  • Have decision-making capacity (or surrogate)
  • Agree to case management services
