Maternal, Paternal, Child, and Adolescent Health (MPCAH) is dedicated to coordinating services for families, including teens, pregnant women, mothers, fathers, caregivers, and children in Alameda County. MPCAH enhances access to comprehensive, quality health care and focuses on early intervention and prevention services. The goal is to reduce health disparities, and protect and improve health outcomes among Alameda County families, including pregnant women, parenting women and men, and their children.

MPCAH programs work to ensure that women, children, youth, fathers, and families achieve optimal health and well-being through the provision of client-centered, culturally responsive, strengths-based services and supports, combined with community transformation efforts that aim to improve neighborhood conditions.

Our target audience is families — women, men, and children — who reside in Alameda County.

Needs Assessment

Every five years California is required to complete a comprehensive needs assessment on the health and well-being of the population we serve. Each local health jurisdiction (LHJ) completes a local needs assessment every five years and uses the results to plan local programs. Black Infant Health is a group-based, social empowerment approach coupled with case management activities for pregnant Black women.

Blue Skies: Mental Health & Wellness Team (MWT)

Blue Skies supports families facing mental health challenges by offering resources, stabilization and linkages to mental health services.

Building Bridges

Building Bridges is a nursing case management program that conducts physical and psychosocial assessment, education, and care planning for at-risk pregnant women and their families by Public Health Nurses.

Desired Reproductive Health Access for Maternal Services (DREAMS)

DREAMS program provides health linkages and assists with access to care for underserved women who have experienced the tragedy of losing a baby either during pregnancy or at birth.

EmbraceHer

Fatherhood Initiative

The Fatherhood Initiative promotes responsible fatherhood and healthy relationships through case management, care coordination, parenting education groups, and skill building.

Fetal and Infant Mortality Review (FIMR)

The FIMR program in Alameda County gathers data on fetal and infant loss, sharing it with providers to analyze and make community, behavioral, and healthcare system improvements.

Hayward Promise Neighborhood

Hayward Promise Neighborhood targets clients who live in Downtown Hayward or have a child who attends a target school: Burbank, Cherryland, Fairview, East Avenue, Harder, Bret Harte, Chavez, Hayward High, Tennyson High and Brenkwitz Alternative).

Healthy Families Alameda County

Healthy Families Alameda County is an evidence-based, comprehensive, intensive home visiting program by trained social workers.

Nurse Family Partnership (NFP)

Nurse Family Partnership is a home visiting program that partners nurses with first-time moms, in order to give babies the best start in life.

Perinatal Services & Reproductive Equity Program

The Perinatal Services and Reproductive Equity team works to improve access to and provision of respectful equity-focused care for marginalized populations in Alameda County who have the highest rates of disparate birth outcomes such as preterm birth, hypertension, diabetes, and infant mortality.

Preconception and Reproductive Health

The Preconception and Reproductive Health program promotes positive sexuality and empowers people to decide if, when, and how many children they want to have, including pregnancy prevention and birth spacing.

Special Start

Special Start is a long-term relationship-based family support program. A multidisciplinary team provides home visiting services. It is designed to address the health, developmental and social support needs of a select population of families whose babies have been hospitalized in an Alameda County neonatal intensive care nursery.

Sudden Infant Death Syndrome (SIDS) and Sudden Unexpected Infant Death (SUIDS) Program

The Alameda County SIDS program coordinates and reconciles reported cases, and promotes SIDS education and training.
