Message from the Director
We, as a system and as a community, are currently poised to make a significant difference in the serious challenges that Alameda County is facing. Since 2016, Alameda County has received $168 million across six years from the California Department of Healthcare Service to implement a whole person care pilot to “coordinate health, behavioral health, and social services in a patient-centered manner with the goals of improved health outcomes and more efficient and effective use of resources.”
Our approach is to strengthen our service system so that our consumers’ needs are met by placing the consumer in the center, the place where solutions and possibilities begin. This idea, this approach aligns with many partners and organizations throughout Alameda County and we now have the resources and partnerships to make this vision a reality.
